Наблюдаемая вариабельность антителообразования и чувствительности серологических методов затрудняет объективную оценку сероконверсии в педиатрической популяции.</p><sec><title>Цель</title><p>Цель: определить качественные и количественные показатели антител классов IgM и IgG к SARS-CoV-2 у детей с соматической патологией в остром периоде COVID-19 в условиях циркуляции варианта Omicron.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы: проспективное одноцентровое нерандомизированное когортное исследование у госпитализированных детей с подтвержденным COVID-19 и сопутствующими хроническими заболеваниями. Серологическое тестирование выполнялось на 1, 8 и 14 сутки болезни методами ИФА и ИХЛА для определения уровня IgM и IgG.</p></sec><sec><title>Результаты</title><p>Результаты: В анализ включены 99 детей с коморбидной патологией. Частота серопозитивности по IgM была низкой на всех сроках (до 26,3% на 14 сутки). IgG выявлялись у большинства пациентов уже на 1 сутки, при этом ИФА демонстрировал более высокую чувствительность (77,6%) по сравнению с ИХЛА (60%), максимальный уровень IgG регистрировался на 8 сутки болезни с последующим снижением на 14 сутки (p &lt; 001). У детей с патологией дыхательной системы уровень IgG был статистически выше, чем у пациентов с гемобластозами, солидными опухолями, ПИД и болезнями нервной системы.</p></sec><sec><title>Выводы</title><p>Выводы: Серологическая диагностика COVID-19 у детей с коморбидностью требует учета метода тестирования, сроков болезни и характера соматической патологии. IgG являются более информативным маркером, чем IgM, с наибольшей чувствительностью при использовании ИФА. </p></sec></abstract><trans-abstract xml:lang="en"><p>Тhe characteristics of the humoral immune response to SARS-CoV-2 in children, particularly those with comorbid conditions, remain incompletely understood. The observed variability in antibody production and the sensitivity of serological assays complicate the objective assessment of seroconversion in the pediatric population.</p><sec><title>Objective</title><p>Objective: To determine the qualitative and quantitative profiles of IgM and IgG antibodies against SARS-CoV-2 in children with somatic comorbidities during the acute phase of COVID-19 amid the circulation of the Omicron variant.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods: A prospective, single-center, non-randomized cohort study was conducted involving hospitalized children with confirmed COVID-19 and underlying chronic conditions. Serological testing was performed on days 1, 8, and 14 of illness using enzyme-linked immunosorbent assay (ELISA) and immunochemiluminescence assay (ICLA) to measure IgM and IgG levels.</p></sec><sec><title>Results</title><p>Results: A total of 99 children with comorbidities were included in the analysis. The seropositivity rate for IgM remained low at all time points (up to 26.3% on day 14). IgG antibodies were detected in the majority of patients as early as day 1, with ELISA demonstrating higher sensitivity (77.6%) compared to ICLA (60%), peak IgG levels were observed on day 8, followed by a significant decline by day 14 (p &lt; 0.01). Children with respiratory system diseases exhibited statistically higher IgG levels compared to those with hemoblastoses, solid tumors, primary immunodeficiencies, and nervous system disorders.</p></sec><sec><title>Conclusions</title><p>Conclusions: Serological diagnosis of COVID-19 in children with comorbidities requires careful consideration of the testing method, disease timeline, and type of underlying somatic pathology.
